The SBA has released 3 types of PPP forgiveness application forms, which were all updated in January 2021 and can be found here:
- Form 3508S — This is the SBA's simplified forgiveness application that First or Second Draw borrowers with loans with principal amounts of $150,000 or less are eligible to use. Borrowers using Form 3508S are not required to submit documentation for their expenses. Borrowers with Second Draw Loans using Form 3508S will need to submit revenue numbers and documentation to satisfy the 25% or more revenue reduction requirement they attested to when they received their loan.
- Form 3508EZ — This is a more extended forgiveness application form that can be used by First or Second Draw borrowers who can attest that they 1) did not reduce salary/wages by more than 25% and did not reduce their number of employees OR 2) did not reduce salary/wages by more than 25% and experienced a certain decline in business activity due to COVID-19.
- Form 3508 – This is a more extended forgiveness application form to be used by borrowers who do not qualify to use either of the forgiveness application forms above — for example, because the borrower has a PPP loan greater than $150,000 and has reduced staff during the relevant reference period.
When you complete the ‘set-up’ section of Scratch’s online forgiveness application form, you will be directed through to the application form that is relevant to you based on your responses to the set-up questions. You will not need to submit a separate PDF forgiveness form – your forgiveness application submission through the Scratch website is the equivalent of submitting that form.
